It’s most likely due to the animation style. I have nothing against that style, but if it works the way I think it does, it’s basically involves animated pictures put together into marionettes. Since this type of style relies on stock images, it can become easy to make the mistake of mirroring a non-symmetrical image (like Kopaka’s mask) instead of putting in a new one.
Agreed, although I vaguely recall something similar to this happening in G1. Anyone remember what it might be?
In MNOG there was a scene in Kini-Nui where Kopaka’s scope was on the wrong side. Eljay brought it up in MNOG with Meso, so that may be where you got it from.

This is great

Well, there you go…
Poor Kopaka, first he slips, then his eyes get switched around…
Ooh, I knew Soundwave was a traitor.
Really, her powers are all over the place, huh? Now she can have her hair faze through her body?
Maybe she can change the density of Ice so it can pass through stuff, and she froze her hair…
Suprisingly, no one has posted any B:TAS errors here.
For some reason Bing images isn’t working, so…
That seems more like the style. We have seen many times that the Toa (and other characters) have floating limbs.
Aw I was gonna do that!
Not technically an “animation” error, but cell dirt created a white mole above his upper lip in the intro to the show.
Behold! The RWBY intro, now red and upside-down.
This is technically a “smear” but since they probably didn’t mean to have this scene overlaid on Ruby’s eye (or upside down, for that matter) I put it here.

In the episode of Phineas and Ferb when they’re trying to find crikey the crocodile Irving was pressing buttons on his tablet but the layering was messed up so his hand was in front of the tablet
