Question regarding Prototype's fusion?

To prove it, he tracked down a Toa of Fire and a Toa of Earth and blasted them just as they were merging their forms. But instead of destroying them, the weapn fused the two Toa together permanently and drove them mad. The result, Prototype, went on a rampage and then disappeared for several centuries”

For context, the ‘weapon’ here is the Spear of Fusion later wielded by Vezon.

• This statement makes it sound like they were trying to form a Kaita-like fusion of two Toa. Is this assessment correct? If not, what is implied here?
• If that is correct, is it possible for two Toa to fuse on their own, or would their attempt have failed? Previously you have stated a voluntary fusion to require at least 3 beings.

2) For a voluntary fusion, yes. Spear of Fusion could merge two Toa.
