One mystery we never got a full answer to was who on the Red Star decided to transform the Inika. Was it one of the Kestora, or is there someone else up there in charge of these decisions?

It was an automatic failsafe, likely put into place to help fix the MU in the event that it was breached and in a severe state of disrepair, by creating Toa if none were already nearby.
So in short, nobody. It was simply designed to do so on its own by the Great Beings.
